How do I verify if my insurance is accepted?
Call our office at (203) 535-0206 with your insurance information, and our staff will verify your coverage and benefits before your appointment. We recommend doing this at least a few days before your scheduled visit.
What should I bring to my first appointment?
Please bring your insurance card, a valid photo ID, a list of current medications, any relevant medical records or imaging (MRI, X-ray, CT scan), and your completed new patient paperwork. Arriving 15 minutes early allows time to complete any remaining forms.
Do I need a referral?
This depends on your insurance plan. Some plans (like HMOs) require a referral from your primary care physician, while PPO plans typically do not. Please check with your insurance provider or call our office to confirm.
What if I don't have insurance?
We offer self-pay options for patients without insurance. Please contact our office to discuss pricing and payment plans. We believe everyone deserves access to quality pain management care.
Will my insurance cover the procedures?
Coverage varies by plan and procedure. Many interventional pain management procedures are covered when deemed medically necessary. Our staff will work with your insurance company to obtain pre-authorization when required and will inform you of any estimated out-of-pocket costs.
What is your cancellation policy?
We require at least 48 hours notice for appointment cancellations or rescheduling. Late cancellations or no-shows may result in a fee. We understand emergencies happen - please contact us as soon as possible if you need to change your appointment.
